What the booking form collects
- First name, last name and phone number (required).
- Email address (optional).
- Optional treatment-interest checkboxes: orthodontics (aligners, braces); kids (preventive care, laughing gas); services (fillings and decay, crowns and bridges, porcelain and zirconia, root canal treatment).
Treatment interest is health-related information. Please do not type medical details beyond what is needed to book.
Why we collect it and where it goes
The clinic uses these details to respond to your appointment request.
The booking page embeds a GoHighLevel (LeadConnector) form. Submissions go into the clinic's GoHighLevel account and do not pass through this website's own code.
The form uses Cloudflare Turnstile as an anti-bot check.
Visit measurement and browser storage
The site loads the Meta pixel (ID 1339003018247792) and its PageView event on every page to measure visits to the site and the performance of the clinic's ads. The Meta pixel sets its own cookies.
The site's typefaces are loaded from Google Fonts (fonts.googleapis.com and fonts.gstatic.com). When a page loads, your browser requests these files from Google, which receives your IP address as part of that request.
The site uses two sessionStorage keys, idj_ad_code and idj_utm, to hold which ad or campaign brought you to the site. This information is cleared when the tab closes. When you open the booking page, these values (the ad code, campaign parameters and Facebook click ID, if present) are added to the booking form's web address, so the clinic's GoHighLevel booking system receives them together with the form, so the clinic can see which ad or campaign led to a booking request. Closing the tab clears the browser copy; it does not remove what GoHighLevel has received.
Website hosting
The site is hosted on 313's own web server, using nginx. Cloudflare provides DNS for the site.
